IDAHO SOCIETY OF HEALTH-SYSTEM PHARMACISTS Privacy Statement

 

BACKGROUND

The Idaho Society of Health-System Pharmacists (ISHP) has created this statement to help you understand how your personally identifiable information is collected and may be shared. The following discloses the information gathering and dissemination practices for this website.

COOKIES

The ISHP website uses cookies to keep track of your Browser type, IP address, access time, and files accessed.  This information is collected in log files. ISHP uses this information to analyze traffic on the site. Aggregate information generated from the log files may sometimes be shared with advertisers. ISHP members who elect to browse the site with the cookies option turned off may not be able to access certain restricted content.

 

PERSONALLY IDENTIFIABLE INFORMATION

The site collects personally identifiable information at some locations. The information collected may include your name, title, employment, address, telephone numbers, and e-mail addresses. We may use this contact information to send newsletters, event announcements, or for other association purposes. Users may choose to decline receiving future mailings; please see the "Opting Out" section below. Certain aggregated demographic information may also be compiled and shared in a manner that does not identify the individual user.

 

MEMBERSHIP INFORMATION

Personally identifiable membership information is available to all ISHP Members in the Member Directory, which is in the password-protected Member-Only section of the Website.  This  membership information should be used only for ISHP or professional (pharmacy) related purposes, and not for any illegal, abusive or commercial purposes.  Membership information may also be made available to other interested persons, organizations and/or business, but only on approval of the Board. Any member, person or business who misuses or abuses this information may be banned permanently from further use.  Any Member can edit their profile to exclude some or all of their personally identifiable data from view on the website. 

 

LINKS

This site contains links to other sites. ISHP is not responsible for the privacy practices or the content of other Web sites. Please read the privacy statements of the sites to which we link as their privacy practices may differ from ISHP.

 

SURVEY INFORMATION

Personally identifiable information is optional in our online surveys and would be used only to contact somebody to clarify an answer. Demographic and profile data that are collected may be shared with advertisers on an aggregate basis.

 

E-MAIL

Throughout this site, users are provided the opportunity to communicate with ISHP via email. We read every message and try to reply promptly to everyone. This information in your e-mail is used to reply directly to your questions or comments..  If we feel the information is of interest to other members, and not embarrassing to you personally, we may share your information with others.

 

PUBLIC FORUMS

This site makes discussion boards or listserv subscriptions available to its users, please remember that any information that is disclosed in these areas becomes public information. You should exercise caution when deciding to disclose any personal information. These forums may be moderated and inappropriate messages may be removed.

 

Guidelines for Posting Information in Blogs or Discussion Groups

ISHP has adopted the following guidelines for posting information in Blogs or Discussion Groups:

 

1.    Potentially libelous statements or damaging innuendo, such as accusing somebody of a crime, defaming someone's character, or making statements that can harm somebody's reputation.

2.    Obscene, explicit, or racist language.

3.    Personal attacks, insults, threats, harassment, or posting comments that incite violence.

4.    Comments using another name to disguise your identity.

5.    Commercial product promotions.

6.    Comments unrelated to the story.

7.    Links to other Web sites.

 

While ISHP will not edit a story or comments, we reserve the right to remove material that violates our code of conduct, and repeated abuse of privileges will result in banishment. 

 

If you feel someone has violated our posting guidelines please contact us immediately so we can remove the post. We appreciate your help in regulating our online community.

 

OPTING OUT

Our site provides users the opportunity to opt out of receiving electronic communications from ISHP.  If you do not want to receive electronic communications, you should edit your profile to opt out of this feature.  In doing so, you will not receive many communications from ISHP.

 

CHANGING INFORMATION

Users have the ability to change information previously provided such as name, address, telephone, and e-mail address.  To change your information, please edit your profile.
